Volunteer Application - Ages 18 and above - To start the process of being considered for membership in the Hazlet First Aid & Rescue Squad please fill out the membership inquiry form below, it will be forwarded to the squad liaison and you will be contacted by a member of our squad.

A full application must be submitted to the squad before joining. Click on the following link for a  Fillable Application  or you can go to the files section of the website under the Links and Documents dropdown list for a Microsoft Word or PDF version of the application. Once completed the application can be submitted to the squad by email at joinus@hazletfirstaid.org, through the liaison or by mail to PO Box 162 Hazlet, NJ 07730
